MY INTERNSHIP JOURNEY IN GENERAL MEDICINE
I'm an intern, posted in general medicine department for 2 months including 2 weeks of psychiatry.
First 2 weeks:
Posted in Department of Psychiatry:
Attended the OPD in department of psychiatry and observed various spectrum of psychiatric disorders in the patients. Studied symptomatology regarding various psychiatric disorder spectrums.
I got to know about the ICD-10 Classification of psychiatry disorders and also DSM Classification of diseases.
Done night duties for observation of psychiatry patients of withdrawal syndromes. And learnt how to manage them.
Attended the De addiction centre visits and observed the various types of medical, behavioural therapy going on there.
Learnt how to attend rounds for the patients and how to write repeats in the casesheets from post graduates.
Typed, studied and submitted the discharge summaries of various patients under department of psychiatry. From this I learnt how to discharge a patient with proper counselling required and proper instructions for medications for both patients and attenders.
4 WEEKS OF UNIT DUTIES:
I was posted in the unit-I in the successive 4 weeks.
My first day in unit was our op day. I learnt how to take op cases with proper history and examination and how to investigate and how to manage the treatment part with proper counselling.
Admitted patients who required further management under supervision.
All investigations required and recommended by post graduates, senior residents and faculty had been sent and samples drawn, collected and sent for laboratory.
Studied venous sampling. Performed venous sampling.
Learnt ABG sampling and learnt ABG interpretation from post graduates. Clarified doubts.
Learnt basics of ECG. Interpretation of chest x ray.
Foleys catheterization done for 5 patients.
Ryles tube insertion done for 4 patients.
Completed fever charts in the casesheets and filled investigation charts up to date everyday.
Updated SOAP NOTES for every patient in their PAJR groups created by us.
Created PAJR groups for every patient admitted
Created blogs of all patients with consent from patients and attenders and proper deidentified clinical images.
Blogs created under my blogspot address are as follows:
hypoglycemia secondary to sepsis
Discharged the patients and learnt how to prescribe discharge medications to them.
In total
5 unit op duties including casualty were done.
1 rotational Sunday duty was done.
EXAMINATION DUTY:
Attended University practicals of juniors 2018 regular batch and contributed my efforts in conducting their examination.
Found spotters for the juniors from their blogs.
REFERRALS:
attended referrals came for general medicine of my unit and learnt the importance of general medicine and the need of inculcating my knowledge in all aspects of health science
LAST 2 WEEKS OF PERIPHERALS:
FIRST 5 DAYS: ICU AND AMC DUTIES.
Monitoring hourly for ventilator patients.
Learnt ventilator settings.
Expertised in ABG SAMPLING.
Learnt ryles feeding.
Helped my junior interns in venous sampling, ABG sampling etc
6 Days of nephrology:
Admitted patients. Sent investigations, discharged patients.
Monitoring vitals for ward patients and half hourly for dialysis patients.
5 DAYS OF WARD:.
Posted for 5 days.
Monitoring and it's importance.
Made all unit interns to update soap notes in the respective pajr groups.
Made blogs and pajr groups of nephrology patients.
Made good friends in the department. Thanks for everything
